Be sure to bring any diagnostic films or other test results that may have been ordered or performed by another physician.
Please bring the following items to your first visit at Orthopaedic Associates:
- Photo ID (Driver's License, State ID, etc.)
- Insurance cards
- Any outside films and/or diagnostic reports
- Previous lab results
- Medication list including strengths/dosage
New Patients
Once you have made your initial appointment, you may pre-register by downloading, printing and completing the forms below. Please be sure to bring them with you to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

Established Patients
Any registration and history information can now be entered or updated via our Patient Portal. If you have not activated your Patient Portal account, please do so as soon as possible. If you do not have an activation code, contact our office and we will provide you with your unique code and the necessary steps to set up your Patient Portal account. Entering this information prior to your visit will save you time in the office.
