Once you have made your initial appointment, you may download, print, and complete the forms prior to your arrival.  This well help to speed up the new patient registration process and get you in to see the doctor faster!

Please bring the following items to your first visit at Orthopaedic Associates:

• Any outside films and/or diagnostic reports
• Lab results
• Insurance cards
• Medication list including strengths/dosage
• Printed and completed Patient Registration Form (if not submitted by computer)
• Printed and completed Health History Form
• Printed and completed Privacy Notice Form
